Physical Therapist (PT) for Home Health

FeldCare Connects is seeking a self-motivated Physical Therapist to deliver premier excellence of care to adults of all ages in the Moorpark area. The Physical Therapist will assess and evaluate patient care needs, determine treatment plans, and monitor progress. The role requires knowledge of physical therapy principles, effective communication skills, and the ability to prepare records with accuracy and discretion.
