Job Description
The Enhanced Care Management (ECM) Program Manager oversees the implementation of the ECM program for young children and families with complex medical and social needs, ensuring alignment with CalAIM requirements and regulatory compliance.
Requirements
- 5+ years of experience in care management, case management, or health/social service delivery
- 3+ years of program management and supervisory, administration, or team leadership experience
- Master's degree in social work or a related health field
- Current and active License in one of the following: Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC)
