Referral Coordinator - Eastern PA

Forge Health is a mission-driven outpatient mental health and substance use provider seeking a Referral Coordinator to build partnerships and drive awareness of their behavioral health services across Eastern PA. The role requires regular local travel, onsite engagement with referral partners, and active participation in community events. Incentives are tied directly to referral growth and relationship performance.

Requirements

• 2+ years prior experience in behavioral health, Healthcare outreach, provider relations, or business development
• Working knowledge of behavioral healthcare models, referral processes and provider workflows
• Availability and flexibility to work outside standard 9–5 hours as needed
• Experience building and maintaining professional relationships with clinicians, practices, or healthcare organizations
• Familiarity with CRM systems and comfort managing outreach pipelines and activity tracking
• Valid state license and access to a vehicle
