Endocrinology and Obesity Medicine Physician - Dedicated Collaborator W2 Telemedicine

Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. We are seeking a dedicated and collaborative Physician specialized in Endocrinology and Obesity Medicine to join our innovative, multidisciplinary chronic disease management team.

Requirements

• Board certification in Endocrinology by the American Board of Internal Medicine (ABIM)
• Highly Preferred: Certification by the American Board of Obesity Medicine (ABOM) or demonstrated extensive clinical experience (minimum 3 years) in obesity medicine and pharmacotherapy (older generation oral and newer injectable medications).
• Minimum of 5 years as an attending physician in endocrinology.
• Demonstrated competency and comfort in managing patients with complex co-morbidities, including CKD, ESRD, and CAD/CV conditions.
• Expertise in advanced diabetes management, including the use of CGMs, insulin pumps, and complex insulin regimens.
• Strong interest in mentoring, teaching, and collaborating with Endocrine-Specialized APPs and Registered Dietitians specializing in diabetes.
• Demonstrated experience working with APPs, medical residents, or trainees in a clinical academic setting.
• Excellent communication and interpersonal skills.
• Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for both endocrine disorders and obesity.
