Case Manager

The Case Manager is responsible for the day-to-day activities of their assigned caseload, including screening, assessing, counseling, and linking homeless, formerly incarcerated residents with histories of mental illness, substance use, and/or HIV/AIDS to needed permanent housing.

Requirements

• High school diploma or GED
• 2 years of experience working with HIV/AIDS, substance abuse, and/or criminal justice issues
• Experience working with clients, at-risk youth, and other disenfranchised populations
• Microsoft Office skills and database case management system experience
