Social Care Navigator

The Social Care Navigator serves as an entry point for the delivery of HRSN services to The Fortune Society clients, engaging Medicaid participants to identify health-related social needs and connect them with appropriate community-based care.

Requirements

• Bachelor's degree in community health and/or equivalent
• Minimum of 2 years' experience in a similar role
• Bilingual or multilingual preferred
• Computer skills and ability to navigate analytic technology solutions for documentation, tracking, and review of data
• Strong leadership and management skills
