Job Description
The Case Manager is responsible for the planning and treatment of individual children, families, or assigned groups, and will perform a variety of tasks such as developing care plans, participating in documentation, and providing a supportive atmosphere for the youth.
Requirements
- Bachelor's Degree in social work or a degree in a human services related field
- Two years of experience in child welfare or a Master's Degree in Human Services
- Prior experience in working with Foster and Adoptive families
- Self-driven/reliant and able to work independently as well as part of a team
- Strong organizational skills
- Empathy/compassion to build trust based relationships
- Effective verbal and written communication skills
