Job Description
Gateways Hospital and Mental Health Center seeks a motivated Case Manager to assist clients in re-integrating into the community through benefit establishment, resource access, and skill development. The Case Manager collaborates with the treatment team and residential staff to ensure quality care for clients. The role involves securing benefits, coordinating medical care, transporting clients, and documenting services.
Requirements
- Prior case management experience
- Academic training in Behavioral Sciences
- Experience with the mentally ill and/or forensic populations
- Valid California Drivers license
- Driving record acceptable for coverage by Gateways insurance carrier
