Job Description
The RN Care Coordinator leads GCHC's disease management and disease prevention programs, providing assistance to providers and working directly with patients. The position involves developing individualized care plans, ensuring quality care using evidence-based guidelines, and promoting efficiency in healthcare delivery.
Requirements
- Graduation from an accredited school of Nursing
- Current license as a registered nurse (RN) in the State of Michigan
- Ability to move up to 30 lbs. in order to transport supplies/equipment and to move or position patients
- Two years working for a physician or other outpatient health center/clinic setting
- Proficiency with Electronic Health Records
