Job Description
As an ambulant begeleider Bemoeizorg, you will work in a composite team with GGZ, addiction care, GGD, and social care, providing support to people with complex problems in mental and physical health, addiction, and social circumstances. You will be responsible for actively making contact with applicants and clients, breaking down care avoidance, and guiding clients to suitable assistance. You will work in a multidisciplinary team and have the flexibility to have more impact.
Requirements
- Completed HBO education in Social Work (with exit profile Care), SPH, Registered Nurse level 5/6 or MBO Registered Nurse level 4 with BIG registration
- Experience in ambulant guidance of the EPA target group, think of experience in FACT-teams
- Own vehicle and driver's license
