The Behavioral Health Therapist provides comprehensive assessment, diagnosis, consultation, and interventions for behavioral health, and/or substance use needs, and/or social determinates of health needs. The role involves counseling/behavioral health services to patients on an individual, couples, family, and/or group basis, as well as assisting the care team in developing care management processes to improve self-management of chronic conditions. The therapist will also consult and collaborate with patient and care team regarding the recommended behavioral health intervention(s).
