About Grace Community Care. We are seeking licensed clinicians who are passionate about serving diverse and underserved populations. As we integrate behavioral health services into our existing community support model, we are looking for compassionate and clinically grounded professionals to provide comprehensive behavioral health services to adults and families in outpatient and community-based settings.

Requirements

• Master’s degree in Social Work, Counseling, or related clinical field
• Active New Jersey license: LCSW, LPC, LAC, or LSW
• Active Type 1 NPI number
• NJ Medicaid enrollment
• CAQH profile
• Professional liability insurance
• Strong understanding of HIPAA and clinical documentation standards
