Case Manager | PRN | Care Continuum

Delivers care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation in coordination with other health team members. Provides high quality care to patients experiencing a life limiting illness.

Requirements

• Accurate and timely record patient psychosocial assessments and interdisciplinary team meetings
• Recognize opportunities for documentation improvement and collaboration for plan of care
• Effective and appropriate communication with providers, nursing and ancillary staff, and leaders
• Ability to multi task and manage multiple priorities
• Accurate input of data and review of quality metrics
• Demonstrate an understanding of medical complications, co-morbidities, severity of illness, risk of mortality, secondary diagnosis, and an ability to impart this knowledge to providers and other members of the interdisciplinary team
• Evaluate nursing intervention outcomes, conferring with other health care team members as necessary
• Gather and analyze information pertinent to documentation findings and outcomes
• Timely follow up of culture results and commmunication
• Understanding of social determinants of health screening and assisting the interdisciplinary team with communication of resources to the patient
• Ability to manage patient's health concerns utilizing technology
• Regular and reliable attendance
