Delivers care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation in coordination with other health team members. Provides high quality care to patients experiencing a life limiting illness.
Requirements
- Accurate and timely record patient psychosocial assessments and interdisciplinary team meetings
- Recognize opportunities for documentation improvement and collaboration for plan of care
- Effective and appropriate communication with providers, nursing and ancillary staff, and leaders
- Ability to multi task and manage multiple priorities
- Accurate input of data and review of quality metrics
- Demonstrate an understanding of medical complications, co-morbidities, severity of illness, risk of mortality, secondary diagnosis, and an ability to impart this knowledge to providers and other members of the interdisciplinary team
- Evaluate nursing intervention outcomes, conferring with other health care team members as necessary
- Gather and analyze information pertinent to documentation findings and outcomes
- Timely follow up of culture results and commmunication
- Understanding of social determinants of health screening and assisting the interdisciplinary team with communication of resources to the patient
- Ability to manage patient's health concerns utilizing technology
- Regular and reliable attendance
