Job Description
GuideStar Eldercare is seeking a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to provide neurobehavioral health services to residents in long-term care facilities. This role involves providing evaluation, condition-specific support therapy, and collaboration with specialty clinicians to ensure holistic care for elderly individuals with neurocognitive disorders and/or other neurologic/psychiatric conditions. The position is fully in-person and requires an interdisciplinary team approach.
Requirements
- Ability to work as part of an interdisciplinary treatment team
- Good time management skills
- Able to function autonomously with regional Area Manager support
- Current licensure as an LMHC or LCSW in the state of application
- Bachelor’s degree in Mental Health Counseling or Social Work
- Passionate about geriatrics
- Experience with brief bedside assessment screens
- Motivated by productivity-based positions
- Ability to navigate and document on an Electronic Medical Record (EMR) system
