Patient Care Coordinator

The Patient Care Coordinator will provide community case management, including providing linkages and resources to patients. They will work in close collaboration with clinicians to assess patients' needs, provide resources, and improve patient health outcomes. The role is funded through the CITED grant and is approved through December 2026. Continued employment is contingent upon Harbor Community Health Centers securing continued funding.

Requirements

• High School Diploma or equivalent/AA degree
• Minimum 2 years of experience in intensive case management or social services field
• Bilingual English/Spanish required
• Demonstrated commitment to working with diverse communities
• Strong verbal and written communication skills
• Patient care-centered mentality
• Adept computer and internet skills
• Experience with Microsoft Word and Excel Spreadsheets
• Proactive problem-solving skills
• Excellent time management skills
• Ability to maintain confidentiality and handle sensitive information
