Requirements

• Minimum 2 years Functional Medicine experience required working in Illinois, after Board Certification
• Must be Board Certified
• Licensed to practice in Illinois
• Experience in outpatient primary care strongly required
• Certification/Background in integrative care, functional medicine required
• Past work experience in a medical wellness clinic required
• Excellent professional/academic references and credentials
• Comfortable with EMRs and computers
