The Wound Care RN- Case Manager provides patient care and handles intake and discharge processes for the Wound Care Center patients, and may also serve as a Case Manager to a group of assigned patients. The role requires a Registered Nurse with a degree in Nursing and current RN license in the state of practice. The position involves providing patient care, handling patient intake and discharge processes, and collaborating with other healthcare providers. The role also requires strong interpersonal and communication skills, as well as analytical and problem-solving skills. The Wound Care RN- Case Manager works in a patient care environment and is required to perform tasks such as lifting and moving items, and working with computer systems.
