Job Description
The Care Coordinator provides counseling, educational, and referral services to enrolled patients and their families, conducts client assessments, and formulates treatment plans in adherence to State opioid treatment program regulatory requirements.
Requirements
- Bachelor's degree
- Valid Indiana driver's license
- FSSA/DMHA-approved credentials for providing counseling with an OTP
