Job Description
This is a rewarding Community Care Coordinator position for experienced healthcare professionals. The role focuses on assessing patient care needs, connecting patients with services, and providing person-centered care in a supportive environment. Requirements
- Membership in the applicable regulatory body (College of Nurses, Physiotherapists, Occupational Therapists, etc.)
- 1+ year of recent experience in community health or a related field
- Knowledge of the health care delivery system and community resources
- Excellent interpersonal, communication, assessment, problem-solving and decision-making skills
- Valid driver’s license and access to a reliable vehicle
- Proficiency in a Windows environment
