Care Coordinator

Join Neighborhood as a Care Coordinator and play a crucial role in creating a positive experience and improving health outcomes for patients. As a care coordinator, you'll work with care teams to listen with kindness to patients' concerns and seek solutions, assist patients and families with navigating services and community resources, and perform administrative tasks.

Requirements

• High school diploma or equivalent required; associate's degree, preferred
• Experience in community health, patient resource coordination or equivalent
• Computer skills, clerical skills, data entry and organization
• Excellent written and verbal communication, and problem solving skills
• Able to read, write and speak the English language. A second language of Spanish is preferred
