Job Description
Care Coordinator responsible for providing patients with appropriate referrals, coordinating specialty care, and scheduling appointments. Must interact positively with staff, patients, and community members.
Requirements
- Medical Assistant Certification is a plus
- High School Diploma or GED required
- 2-4 years of medical office experience in an out-patient clinical setting
- Bilingual in Spanish OR Khmer (strongly preferred)
