Job Description
We are seeking a Care Coordinator to provide patients with referrals to internal/external resources and agencies, coordinate specialty care, and serve as a liaison between healthcare staff. The ideal candidate will have 2-4 years of medical office experience, excellent organization and communication skills, and be bilingual in Spanish or Khmer.
Requirements
- Medical Assistant Certification (plus)
- High School Diploma or GED
- 2-4 years of medical office experience in an out-patient clinical setting
- Bilingual in Spanish or Khmer (strongly preferred)
