Job Description
We are seeking a Care Coordinator to provide patients with appropriate referrals to internal/external resources and agencies, coordinate specialty care for clinic patients, and schedule appointments for clinic patients with a provider.
Requirements
- Medical Assistant Certification (plus)
- High School Diploma or GED
- 2-4 years of medical office experience in an out-patient clinical setting
- Bilingual in Spanish or Khmer (strongly preferred)
