Job Description
CARE and BE CARED FOR – THIS IS YOUR HOME is a Part Time Team Assistant position at Ontario Health atHome, providing administrative support to team members and ensuring excellent customer service to patients. The role involves processing referrals, managing patient records, and providing back-up support to various positions. It aims to contribute to a collaborative team environment focused on providing connected, accessible, patient-centred care.
Requirements
- Provide administrative support services to Care Coordinators
- Process new referrals, and orders for services, supplies and equipment
- Process and assist in managing confidential patient records
- Enter, update and maintain a high volume of patient data in the electronic database
- Answer a high volume of telephone inquiries from patients, families and service providers, and refer callers as appropriate
- Provide back-up support to other positions, as required
