RN Care Coordinator

Provides a deliberate patient and family centered partnership with the healthcare team to facilitate a seamless patient experience between Inpatient and Outpatient settings. Maximizes patient’s health outcomes and the patient experience by identifying patient and family needs, completing pre-visit planning, supporting the clinic visit and providing closure to the clinic visits.

Requirements

• RN License
• 3 years of clinical experience
• Experience with Chemotherapy and cancer infusions
• EMR experience
• Nursing assessment and evaluation skills and abilities
• Knowledge of standards and clinical practice for area of care coordination
