Job Description
Personal Care Coordinator (PCC) needed to work under the direction of the Supervisor of Case Management. The PCC will support members in completing their health risk assessment (HRA) and ensure communication of the member’s HRA and care plan with the member, Primary Care Provider (PCP), and health care team.
Requirements
- High school diploma or equivalent required.
- 2+ years experience working with the needs of seniors or persons with disabilities (SPD) in a customer/member service capacity.
- Bilingual in English/Spanish, English/Vietnamese, English/Korean, English/Mandarin, English/Arabic or English/Farsi is preferred.
