Job Description
The Social Worker is a behavioral health professional providing outreach to members, assisting in identifying social determinants of health and barriers to care, and facilitating referral to other services through collaboration.
Requirements
- Master's degree in social work
- An active, unrestricted, state board-issued LCSW (Licensed Clinical Social Worker) license
- 3 years of experience in healthcare or human services agency setting
- Experience in a managed care setting
- Experience working in the community with knowledge of renal health and/or community resources
- Must reside in Michigan
