Job Description
The Heartland Homes program provides facility-based Permanent Supportive Housing in Council Bluffs, IA, using a Housing First model to individuals and families experiencing long-term homelessness. The Case Manager will work to assess, verify eligibility, and provide long-term case management/supportive services to clients.
Requirements
- Bachelor's Degree in Social Work or Human Service field
- Valid Driver's License and acceptable driving record
- Two years direct care experience in Human Services preferred
