MDS CARE PLAN COORDINATOR

The MDS Care Plan Coordinator manages the integrity of the facility's RAI and Medicare processes, ensuring residents receive Medicare services and all documentation is completed timely and accurately. Responsibilities include obtaining physician certifications, coordinating MDS and Care Plans, and tracking Medicare resident benefit days.

Requirements

• Current, valid State Nurse license
• Education/experience in long-term care, acute care, or geriatric nursing (preferred)
• MDS/RAI experience (highly preferred)
• Functional literacy in English
• No disqualifying criminal offenses
• Proficiency in computer use/data entry
