Support Coordinator

We're looking for a Support Coordinator to join our team at Home Group. You'll work with customers to create personalized support plans and provide daily support to help them achieve their goals. This is a full-time role with a competitive salary and benefits package.

Requirements

• Passion and experience in supporting others ideally in a supported housing setting
• Experience assessing referrals and coordinating support
• Ability to work under own initiative, remain calm under pressure, and have a resilient approach
