Case Manager

Join Hope Network as a Case Manager and help people overcome challenges. As a talented professional, you will serve individuals across 280 unique locations, providing comprehensive psychosocial assessments, linking consumers to necessary services, and maintaining consumer records.

Requirements

• Licensed Bachelors Social Work (LBSW) or Limited Licensed Bachelors Social Work (LLBSW)
• Licensed Master Social Work (LMSW) or Limited Licensed Master Social Work (LLMSW) preferred
• Limited Licensed Professional Counselor (LLPC) or Licensed Professional Counselor (LPC)
• Temporary Limited Licensed Psychologist (TLLP) or Limited Licensed Psychologist (LLP)
• Bachelor's degree in a Human Service field is acceptable with a Social Service Technician certificate (SST)
• Prior case management experience in a mental health or rehabilitation setting preferred
