Job Description
Hope Services is seeking a Community Support Facilitator to provide support and leadership for consumers to participate in activities and receive services in community settings. The role involves facilitating relationships, skills, and experience in the community, advocating on behalf of consumers, and maintaining professional relationships with consumers and family members.
Requirements
- High school diploma, GED, or equivalent
- Two years of related experience
- Valid California driver's license
- Clear driving record
- CPR and First Aid certification (optional)
