Care Manager II, Specialty Programs (Mercer County)

Horizon Blue Cross Blue Shield of New Jersey empowers our members to achieve their best health. The Care Manager II-Specialty Programs serves as mentor/preceptor/trainer to new care managers in the CCDM department.

Requirements

• License as an RN, or Bachelor degrees in social work, health or behavioral science
• Graduate level degree in Social Work or Health and Behavioral Science (preferred)
• 5 years experience working with special needs
• Strong knowledge of standards of practice for case managers
• Strong knowledge of managed care principles
• Advanced knowledge of specialty population
• Strong organizational skills
• Presentation skills and ability to conduct trainings to teams
• Ability to provide constructive feedback and mentorship to team members
• Strong oral and written communication skills
• Good PC skills and ability to utilize Microsoft Office applications
• Good problem solving and conflict resolution skills
• Car with valid New Jersey State License and Insurance
