RN II - Primary Nurse Care

Horizon Blue Cross Blue Shield of New Jersey is seeking a Registered Nurse (RN) II to perform RN duties for the Primary Nurse population, focusing on member centric care, prevention, and wellness. The ideal candidate will have 2+ years of clinical experience and a strong understanding of case/care/disease management principles.

Requirements

• Assesses member's clinical need against established guidelines
• Develops, coordinates, and assists in implementation of individualized plan of care for members
• Coordinates the delivery of high quality, cost-effective care
• Documents accurately and comprehensively
• Serves as mentor/trainer to new RN's and other staff as needed
