Requirements

• Graduation from a college or university accredited by the American Medical Association with a Doctor of Medicine degree or accredited by the American Osteopathic Association with a doctor of Osteopathy degree
• Successful completion of an approved program of residency
• Active Illinois Physician licensure, plus current DEA registration, Board certification in a specialty area (must be completed within two years of employment)
