Job Description
The HRC Case Manager will support the statewide Harm Reduction Program in county communities by conducting case management, providing outreach and referrals to those in need of services, and offering psychosocial support. This role involves building strong client relationships, managing data, and ensuring the effective delivery of programs.
Requirements
- Provide HRC Case Management in designated county.
- Conduct Community outreach, provider education and client recruitment.
- Offer case management services to address obstacles.
- Refer clients to appropriate resources.
- Follow-up with referrals and clients.
- Prepare monthly reports.
- Maintain client records.
- Maintain client information.
- Attend staff meetings.
- Adhere to Hyacinth policies.
