Care Coordinator

Care Coordinator role at Illumination Health + Home, a non-profit organization addressing homelessness in California. The role involves client-facing work, case management, and advocacy for specific populations. The Care Coordinator establishes relationships with clients, supports their engagement in medical care and social services, and collaborates with the client's Care Team.

Requirements

• Bachelor's degree in social services, Healthcare, or related field
• Experience in homeless services, case management, and mental health support
• Valid California driver's license
• Familiarity with VI-SPDAT
• Knowledge of resources available in Orange, Los Angeles, and Inland Empire counties
• Bilingual in English and Spanish (preferred)
• Proficiency in Microsoft Office Suite (preferred)
• At least 1 year of experience working with at-risk/unhoused individuals (preferred)
• Experience in non-profit housing and/or housing for people with disabilities and chronic health conditions (preferred)
