Nurse Practitioner or Physician Assistant (Primary Care Provider) - PT 20hrs/wk

We are seeking a compassionate and innovative Nurse Practitioner or Physician Assistant (NP/PA) to join our Social Prescribing Whole Person Healthcare Program. This role provides an opportunity to deliver integrated primary care while helping patients connect with community resources and social supports that improve overall health outcomes.

Requirements

• Master's Degree in Nursing from an accredited Nurse Practitioner program or completion of an accredited Physician Assistant program
• Current and valid Nurse Practitioner or Physician Assistant license in the State of Delaware
• Certification and credentials that allow for independent practice as a Primary Care Provider in Delaware
• Minimum of 2 years of clinical experience, preferably in primary care, behavioral health, or integrated care settings
• Prescriptive authority with DEA and Delaware CSR registration
