Intake Coordinator

As an Intake Coordinator at Innovive Health, you will oversee the inbound referral process from first contact to admission. You will collect information from referral sources, process documentation, and collaborate with multiple departments to ensure our patients receive a smooth transition to home care.

Requirements

• Associate’s degree or equivalent in related work experience
• 1+ year experience in a healthcare administrative role
• Knowledge of medical terminology
• Proficiency in Windows environment, with emphasis on Word and Excel
