As a Psychiatric Home Health Nurse, you will provide direct care and case management to patients requiring home care services, working in partnership with the Clinical Director and team. You will assess and evaluate patient status, develop and revise care plans, and communicate with healthcare providers to ensure consistent delivery of care.
Requirements
- Graduate of an accredited school of professional nursing
- Current RN licensure through the State Board of Nursing
- 1-2 years' experience in a home health environment (preferred)
- Prior experience caring for mental health patients (preferred)
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
