Job Description
The Case Manager provides case management and housing stabilization services to homeless Veteran families, including service coordination, outreach, and vocational support. They work to address housing barriers and increase housing stability for participants.
Requirements
- High school degree
- 1 year experience providing direct support services to individuals experiencing homelessness, mental health, substance abuse or other social issues
- Proficient in use of computer and Microsoft office suite (Word, excel, outlook and TEAMS)
- Experience with Homeless Management Information System (HMIS) a plus
