Job Description
Interwell Health is seeking a Renal Care Coordinator (RCC) to support and coordinate the care of patients with late-stage chronic kidney disease (CKD) at local nephrology practices. RCCs will educate patients on treatment options, provide support for transitioning to dialysis, and work with interdisciplinary teams to improve care processes. The RCC will be responsible for a Nephrology Practice, focusing on patient education, and participation in data interpretation.
Requirements
- Minimum of 2 years previous experience in clinical renal patient care.
- Understanding of diabetes and cardiovascular disease processes preferred.
- Strong organizational and communication skills.
