Physical Therapist Home Health

Trio Home Health (Jet Health, Inc.) is seeking a full time, part time or PRN / Per Diem Licensed Physical Therapists to provide PT services to patients in the Flower Mound, Lewisville, TX and surrounding areas. Requirements

• Evaluate patients' physical and functional abilities to develop individualized care plans.
• Plan, implement, and modify physical therapy treatment programs based on patients' medical referrals and assessments.
• Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans.
• Participate in ongoing professional development activities to stay abreast of current trends and best practices in physical therapy.
