Job Description
The CASE MANAGER is responsible for the programmatic component of a facility, implementing and maintaining program requirements, and overseeing individual program plans. They will initiate review, modifications, and bi-weekly reviews, and document all activities in the resident's file. This role involves managing case files, coordinating with community and government agencies, and supporting the director.
Requirements
- A four-year degree in a social, human services, criminal justice or behavioral science program.
- At least one year of experience working in human services, corrections, or community-based services.
- Experience in a related field (work experience equivalent to one year of education) or academic studies in lieu of work experience.
