Job Description
Provide social work/case management services to children who are determined to be eligible for the CLTS waiver and their families. Coordinate services to individuals to receive an array of appropriate services in a planned, coordinated, efficient, and effective manner.
Requirements
- Assessment of children's needs and eligibility for CLTS waiver
- Screening of children to determine eligibility for CLTS waiver
- Development of individualized service plans (ISPs) for children and families
- Coordination of services with family, agency, and community resources
- Mentoring of staff learning the program to complete CLTS functions
