Medical Social Worker

Assists home care patients to cope with their social and health related problems, and provides supportive structure to aid them in obtaining maximum benefits from Home Care and community resources.

Requirements

• Assesses patient's physical, psychological, emotional and social needs
• Provides case work and counseling to assist in resolving problems
• Completes social work assessment and documents plans and goals
• Provides ongoing support to patient/family and significant others
• Makes referrals for outside community support services
• Assists with discharge planning and prepares progress notes
• Maintains confidentiality of patient information
• Participates in patient care conferences and evaluates patient progress
• Identifies need for medical evaluation and interdisciplinary referrals
• Assumes other duties as assigned
