Job Description
Clinical Care Coordinator is responsible for conducting and coordinating skilled services provided to residents, evaluating and updating patient health care plans, and participating in the facility IDT to ensure the resident plan of care is maintained and followed.
Requirements
- Must be a LVN or RN with an active, unincumbered license to practice in CA
- Must have, as a minimum, 2 years of experience in a skilled nursing facility
- Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities
