Job Description
LSF is seeking a talented Case Manager who wants to make an impact in the lives of others. This position will lead case management and other out of home care support activities for youth and their children served through the residential program.
Requirements
- Maintain socially appropriate behavior of youth through sanctioned and designed de-escalation skills and disciplinary procedures
- Support youth in developing transition and permanency plans, including preparation for adulthood, employment, and post-secondary education
- Conducts comprehensive assessments to identify youth strengths, needs, and goals
