Lutheran Services Florida is seeking a talented Early Childhood Service Coordinator who wants to make an impact in the lives of others. The Early Childhood Service Coordinator will provide case management services for at least 30 open cases at any given time with families who have children ages birth to 6.
Requirements
- Planning early childhood events at each Center to engage families and refer to St. Joseph’s Hospital for developmental screenings
- Conducting home visits to meet the needs of the family and ensure successful enrollment in key services
- Working with parents/caregivers to develop individualized family support plan
- Providing families with timely referrals to services
- Partnering with families to build strong parent-child relationships and strengthen their support networks with other parents and community members
- Maintaining regular contact with service providers and documenting service progress
- Providing community education through public presentations
- Organizing, prioritizing, and completing all work assignments by the established deadlines
- Developing and maintaining knowledge of community resources, program eligibility requirements, key contact persons, emergency procedures, and waiting lists of available resources
- Working collaboratively with volunteers and community resources
- Accurately documenting all case activities and maintaining the case file in accordance with the standardized case file format
- Developing and maintaining relationships and partnerships with local programs and offering parents/caregivers information and materials regarding community programs
- Attending all appointments, staff meetings, trainings, seminars, workshops, etc., as necessary and as required by supervisor
- Working flexible hours to include some evenings and Saturdays
- Participating in Center events and working to support contractual outcomes
