Job Description
As a Youth and Family Outreach Case Manager, you will work collaboratively with families to facilitate a team process to give families voice and choice with the outcome of youth with behavioral health issues living in a stable family or community setting.
Requirements
- 3 years of experience in Social Worker or a related field - preferred
- Bachelor's degree in Social Work or related field and/or 3 years of experience
- Experience with youth with behavioral health issues
- Experience working with County systems
- Experience working with culturally and racially diverse populations in lower socio-economic conditions
